Avenues is open and available to existing and new clients during COVID-19. We are offering HIPAA Compliant Video Teletherapy sessions to all and in-person appointments upon request. Contact us and our intake coordinator will help fit you with an experienced and compassionate Clinician.

Make an Appointment- Two Ways

Our Intake Staff will ask you questions to understand your needs and help to match you with an appropriate Clinician. The staff will also send you two emails, one to set up your patient portal account and instructions to download the Breeze App for check-in and self-service on your phone.  The second email will include the individualized link to your Clinician’s virtual waiting room, where you will go to begin your Teletherapy appointment. If you need other options, such as phone sessions or Face-Time, please discuss with Intake.

 

There is still time to receive Mental Health or Substance Abuse counseling with little or no cost to you!
 
Many Insurance Companies: Aetna, Anthem, BCBS, Beacon, Optum & United Healthcare are waiving member responsibility for Teletherapy sessions for a limited time

Please answer the call from (330) 723-7977 to complete the verification. We can only hold your requested appointment for a short time, so if we are unable to get a hold of you, we may have to deny your request. You can submit another request if you do get a denial.

In-Person appointments are available upon request. Please review ourOffice Safety Precautions During COVID-19 Public Health Crisisto understand the responsibilities (yours and ours) if you decide to request an in-person appointment.

 

As a private practice, we do not offer a sliding fee schedule. For Your First Appointment:

  • Download the Breeze app for Apple or Android on your phone to pre-register and check-in your appointment.  For your pre-registration and check-in you will need:
  • Your insurance card and any copay your insurance provider requires
  • Your authorization number is given to you by your insurance company
  • Driver’s license
  • Information about your copayment or deductible. If you do not know this information, please contact your insurance company and ask for an explanation of benefit coverage for mental/behavioral health issues.
  • Your primary care physician’s telephone number
  • Divorce decree or shared parenting agreement for minors of divorced/separated parents

 

Additional Information:

  • Please check-in 15 minutes early.
  • If you have seen a counselor or psychiatrist within the last 2 years, we will need a telephone number for coordination of care.
  • It is helpful for the therapy process if you consider a list of goals for therapy. This will help you and your therapist make better use of the first session.
  • During your first appointment, your counselor will work with you and discuss your concerns to devise a treatment plan and treatment goals.

 

Our goal is for you to be comfortable with your counselor and the treatment you receive at Avenues. If you feel that your counselor is not a good fit, let us know and we will work to find someone different who can better meet your needs.
