It’s time to take your first step on the road to wellbeing. Send us some information about you and what you are looking for so our intake coordinator can help fit you with an experienced and compassionate Clinician who matches your needs.

Complete the fields and request an appointment. We will contact you shortly after receiving your request to get some additional info and confirm your appointment. Please answer the call from (330)723-7977 to complete the verification. We can only hold your requested appointment for a short time, so if we are unable to get a hold of you, we may have to deny your request. You can submit another request if you do get a denial.

As a private practice, we do not offer a sliding fee schedule. Please leave pets at home unless they are a service animal.


First Appointment Please Bring:


  • Your insurance card and any copay your insurance provider requires

  • Your authorization number if given to you by your insurance company

  • Driver’s license
  • Information about your copayment or deductible. If you do not know this information, please contact your insurance company and ask for an explanation of benefit coverage for mental/behavioral health issues. We also do a benefit verification.

  • Your primary care physician’s telephone number

  • Divorce decree or shared parenting agreement for minors of divorced/separated parents


Additional Information:

  • Please arrive 15 minutes early for payments/paperwork.
  • If you have seen a counselor or psychiatrist within the last 2 years, we will need a telephone number for coordination of care.

  • It is helpful for the therapy process if you bring a list of goals for therapy. This will help you and your therapist make better use of the first session.

  • When you arrive, forms are completed and your insurance card is copied.

  • During your first appointment, your counselor will meet with you and discuss your concerns to devise a treatment plan and treatment goals.


Our goal is for you to be comfortable with your counselor and the treatment you receive at Avenues. If you feel that your counselor is not a good fit, let us know and we will work to find someone different who can better meet your needs.
