Your First Appointment

What You Need to Know

As a private practice, we do not offer a sliding fee schedule. We do not participate in Medicaid, Wellcare, or CareSource products.  If this is what you are looking for, please contact Alternative Paths, at (330) 725-9195 or Catholic Charities of Medina at (330) 723-9615 to inquire about their services.

For Your First Appointment:

  • Download the Breeze app for Apple or Android on your phone to pre-register and check-in your appointment.  For your pre-registration and check-in you will need:

  • Your insurance card and any copay your insurance provider requires

  • Your authorization number given to you by your insurance company or Employee Assistance Program if applicable

  • Driver’s license

  • Information about your copay or deductible. If you do not know this information, please contact your insurance company and ask for an explanation of benefit coverage for mental/behavioral health issues.

  • Your primary care physician’s telephone number

  • Divorce decree or shared parenting agreement for minors of divorced/separated parents

 

Additional Information:

  • Please check-in 15 minutes early.

  • If you have seen a counselor or psychiatrist within the last 2 years, we will need a telephone number for coordination of care.

  • It is helpful for the therapy process if you consider a list of goals for therapy. This will help you and your therapist make better use of the first session.

  • During your first appointment, your counselor will work with you and discuss your concerns to devise a treatment plan and treatment goals.

 

Our goal is for you to be comfortable with your counselor and the treatment you receive at Avenues. If you feel that your counselor is not a good fit, let us know and we will work to find someone different who can better meet your needs.
